What is required for a Case Evaluation?

In order to perform a proper Case Evaluation, you must provide

  1. Copies of ALL of your medical records from all doctors and hospitals who have examined or treated you and,

  2. Copies of ALL of your x-rays. It is essential that you provide copies of the x-rays themselves and not of the reports and,

  3. Copies of photographs, after for sure and before if available.

After we have received these, we may request additional information in order to complete the evaluation.
